Please call our office 203.265.3280 between the hours of 8:00am and 5:00pm Monday-Friday to schedule, reschedule or confirm an appointment.
You can also use the online scheduling platform by clicking the button below.

When rescheduling or cancelling, please call at least 24 hours ahead of your appointment. It is recommended that patients arrive 10-15 minutes early for new appointments to allow time for registration and completion of any required forms.

Upon arrival, please remember to bring the following:

  • Drivers license or valid ID
  • Insurance card/ information
  • List of medications
  • Referral letter (if required)
  • Med Pay letter (if required)
