Patient Information

As a new patient, there are a number of important documents that need to be included in your patient file.

To reduce the time you spend in the waiting room at the time of your initial appointment, please take this opportunity to complete the following forms prior to your first visit. Forms are currently accessible through our patient portal.

Billing Information

Insurance

A reality of today’s health insurance system is that most insurance companies do not cover 100% of medical charges. Most patients are responsible for a co-payment or deductible, which is generally billed once we receive your Explanation of Benefits (EOB). You will be asked for your co-pay when you arrive. Please be prepared or we may need to re-schedule. Additional fees may apply if not paid at the time of visit. If your insurance carrier is one with whom we do not participate, you will be asked for payment in full at the time of your visit.

Patients are required to present their insurance card for verification at each appointment. The fees billed to your insurance carrier are based on usual and customary rates, which are competitive with physicians of the same specialty in our area. Insurance carriers, however, have their own methods of determining standard “allowable” charges, which at times are lower. Patients should check to see if their policy requires a referral before scheduling an appointment. If you do not have a referral or pre-certification in place upon arrival, you may be held responsible for payment or your appointment may need to be rescheduled. Our Physical Therapy department is compliant with Medicaid for patients 18 or younger.

Workers Compensation

Patients are responsible for providing all employment and insurance information when scheduling an appointment. It is required to have authorization from your carrier or employer prior to your visit. If you do not have authorization, we will be more than willing to treat you. However, Comprehensive Orthopaedics will bill your primary health insurance in cases where worker’s compensation denies the claim for any reason. Any balance not paid by the worker’s compensation carrier or primary health plan will be the patient’s responsibility. If you have any questions please call our billing office 203.265.3280x2003.

Appointments

Scheduling

Please call our office 203.265.3280 between the hours of 8:00am and 5:00pm Monday-Friday to schedule, reschedule or confirm an appointment. When rescheduling or cancelling, please call at least 24 hours ahead of your appointment. It is recommended that patients arrive 10-15 minutes early for new appointments to allow time for registration and completion of any required forms.
Upon arrival, please remember to bring the following:

  • Drivers license or valid ID
  • Insurance card/ information
  • List of medications
  • Referral letter (if required)
  • Med Pay letter (if required)

Disability Forms

All forms will be completed within 7-10 business days of receipt. A fee of $15 will be charged to process each form not directly related to physician reimbursement. A fee of $25 is charged for each FMLA packet.

Prescription Refills

Refills may be requested only between the hours of 8am and 3:30pm Monday-Friday by calling 203.265.3280x1141. Please allow 1 to 2 days for processing. Comprehensive Orthopaedics does not accept pharmacy refill requests. If you would like a refill on medication, please let the provider know during your visit or call the aforementioned number. We advise that you check your medication supply before noon on Fridays. No refills will be processed on the weekends or after 3:30pm on weekdays.

Late Policy

Our Electronic Medical Record (EMR), in compliance with the Accountable Care Act, requires that certain information be captured at each visit. We recognize that your time is valuable and we make every effort to run on time. Comprehensive Orthopaedics works hard to honor appointment times and respect patients’ schedules. Patients who do not call and do not show up for appointments are charged a $25 “no show” fee. If you do not show for a procedure, such as an EMG or injection, the “no show” fee is $100 whereas a surgical “no show” fee is $200. Patients who “no show” three or more times in a 24 month period may be dismissed from the practice and may be denied any future appointments.
